Act of Consecration

O Mary, you wished to be called the Mother of Perpetual Help to inspire confidence in me; behold, I, (your name), though entirely unworthy to be counted among the blessed number of your servants, ardently desire to partake in the benefits of your mercy: therefore, I prostrate myself before your throne and consecrate to you my intellect, . . . that it may always think of the love of which you are worthy; I consecrate to you my tongue, that it may praise your exalted virtues and spread your veneration; I consecrate to you my heart, that, next to God, it may love you above all.

Therefore, take me, O great Queen, into the beloved number of your servants, hide me under your protection, hasten to help me in all the needs of my soul, especially in the ultimate danger when I am wrestling with death. O Mother of Perpetual Help, I know that you love me more than I can love myself; therefore, I choose you as Lady and Mistress over all my interests and concerns: dispose of me and all that is mine entirely according to your good pleasure.

O my Mother, bless me and come to me with your mighty intercession; come to the aid of my weakness, so that, faithfully serving you here on earth, I may eternally praise, love, and thank you in the hereafter. Amen.

O Mary, Mother of Perpetual Help, pray for me!

St. Alphonsus, I also choose you as my special patron. I ask you, through your love for Jesus and Mary, take me into the number of your devotees and protect me as your servant. Obtain for me a tender and persistent devotion to Jesus in the Most Holy Sacrament and to Mary, as well as the spirit of prayer and a great zeal for the salvation of souls. O my protector, grant that in all my needs, I may take refuge in Mary. Amen."
